Active Membership

Applicants applying for active membership can register online via the link at the top of the page or can download an application for membership and return it with the appropriate dues payment (see below) to the PVMA office at 8574 Paxton Street, Hummelstown, PA 17036. Active dues amount per month of application:

$295: January 1 – March 31
$221: April 1 – June 30
$148: July 1 – September 30
$74: October 1 – December 31

Academic Active Membership

2012 Dues are $100. Applicants applying for Academic membership can register online via the link at the top of the page or can download an application for membership. The completed application should be mailed to PVMA office at 8574 Paxton Street, Hummelstown, PA 17036.

Associate Membership

2012 Dues are $80. Applicants applying for Associate membership can register online via the link at the top of the page or can download an application for membership. The completed application should be mailed to PVMA office at 8574 Paxton Street, Hummelstown, PA 17036.

Graduate Membership

2012 dues for applicants meeting the requirements for recents graduate status are as follows:

$74 - first 2 years out of school
$148 - third year out of school
$221 - fourth year out of school

Industry Partner

2012 dues are $115. Applicants applying for Industry Partner membership can register online via the link at the top of the page or can download an application for membership. The completed application should be mailed to PVMA office at 8574 Paxton Street, Hummelstown, PA 17036.

Government

2012 dues are $100. Applicants applying for Government membership can register online via the link at the top of the page or download an application for membership. The completed application should be mailed to the PVMA office at 8574 Paxton Street, Hummelstown, PA 17036.

Practice Manager
